Outstanding Young Woman Award Nomination Form

The Bluegrass Junior Woman’s Club (BJWC) has sponsored the Outstanding Young Woman Award since 1975. Candidate nomination is based on contributions to the community through volunteer, professional, and personal involvement for the period January 1—December 31. To nominate a candidate, please complete this form and send it to: BJWC c/o Kathy Young, 2165 Market Garden Lane, Lexington, KY 40509. Nominations must be received by March. The awards ceremony will be in April at our annual Gala. The winner will be named at this event. All nominees and their sponsors should plan to attend.
